CMS’ new ACCESS Model launches a decade-long test of outcome-aligned payments in Original Medicare, relying on remote monitoring, digital care, and interoperable data exchange to improve prevention and chronic disease management. Participation applications for the first performance year are due April 1, 2026, giving organizations limited time to understand what the model demands—and whether they’re ready.

In this session, Arcadia’s Chief Medical Officer, Dr. Luke Hansen, and Arcadia’s head of thought leadership and policy expert, Erica Everhart, will break down the model’s structure, required capabilities, and its broader signal for the future of value-based care. The discussion will cover the model’s emphasis on digital care pathways, patient-reported outcomes, and care-plan exchange through HIEs and trusted networks. We’ll also examine how ACCESS aligns with CMS’ broader interoperability push and related initiatives—such as the Rural Health Transformation Program—and what it means for both payers and providers evaluating participation.
