Arcadia Analytics — powered by AWS — was the foundation for CareMount ACO, a Medicare NextGen ACO, with this post-acute care strategy: building a narrow Preferred Provider Network of high-quality skilled nursing facility (SNF), home health, and other ancillary providers to reduce variability in quality, length of stay, cost, and readmission rates.
Using Arcadia Analytics, CareMount ACO was able to:
- Build 23 high-quality, high-value skilled nursing facilities: CareMount ACO built preferred network of 23 high-quality, high-value skilled nursing facilities supported by embedded care coordination.
- Reduce average length of stay by 4 days: CareMount ACO was seeing 19–47 average length of stays (ALOS) and was able to reduce ALOS by 4 days for preferred network.
