Understanding CMS’ LEAD Model and What It Means for ACOs

CMS has introduced the Long-term Enhanced ACO Design (LEAD), a new 10-year model that represents a structural shift in how accountable care is designed, incentivized, and scaled.

LEAD replaces ACO REACH but goes further, introducing a fixed benchmark that does not rebase, new pathways for smaller and first-time participants, integrated support for high-needs populations, and a longer-term vision for evolving risk adjustment. Together, these changes alter both the financial mechanics and the strategic calculus for organizations participating in value-based care.
