Understanding CMS’ LEAD Model and What It Means for ACOs

CMS has introduced the Long-term Enhanced ACO Design (LEAD), a new 10-year model that represents a structural shift in how accountable care is designed, incentivized, and scaled.
LEAD replaces ACO REACH but goes further, introducing a fixed benchmark that does not rebase, new pathways for smaller and first-time participants, integrated support for high-needs populations, and a longer-term vision for evolving risk adjustment. Together, these changes alter both the financial mechanics and the strategic calculus for organizations participating in value-based care.
A Policy Pulse Pop-Up webinar on Friday, April 10, 2026, at 1:00 p.m. ET will break down how the model works, why CMS designed it this way, and which types of organizations are best positioned to participate.
What You'll Learn:
- Understand how LEAD’s fixed benchmark and “no rebasing” structure change long-term financial incentives compared to MSSP and ACO REACH
- Identify which types of organizations are best positioned for LEAD, including REACH participants, MSSP ACOs, and new entrants to value-based care
- Evaluate the operational and strategic considerations organizations should address now to prepare for LEAD participation, including population strategy, risk tolerance, and infrastructure readiness
