Which patients are most impactable by a care management program?
This is an important but challenging question when it comes to the allocation of resources in population health – of the thousands of at-risk or rising-risk patients in a given population, how can a healthcare system identify the cohort most likely to benefit from intervention by nurse care managers?
At Arcadia, principal data scientist and product manager Michael Simon, PhD has been working with the rest of our team on using social determinant of health information, clinical data, and the Hopkins ACG system to develop a care management impact score.
Dr. Simon will explain more about our work at the upcoming Johns Hopkins ACG System 2018 International Conference, where he will be speaking in a data visualization breakout session.

The Hopkins ACG conference will host hundreds of system users from around the world along the Riverwalk in San Antonio, Texas.
