Arcadia and Client Yakima Valley Farm Workers Clinic Present at OPCA
The Oregon Primary Care Association (OPCA) will host primary care executives from around the country in the second meeting of their Learning Collaborative effort. Arcadia client Yakima Valley Farm Workers Clinic (YVFWC) will present on their Alternative Payment and Advanced Care Model (APCM) transformation journey. YVFWC’s chief executive officer Carlos Olivares, chief medical officer Kevin Heidrick, PA-C, and clinic director Michelle Lehr O’Connell will share the story of the YVFWC/Rosewood Family Health Center in Portland, OR.
Under the directorship of Lehr O’Connell and other YVFWC leadership the Rosewood clinic, often termed Radical Rosewood, tries new and innovative approaches to designing healthcare delivery to address true drivers of health outcomes. A focus on building multi-disciplinary care teams, increasing screening and services for social determinants of health, and developing robust analytics and reporting help them to demonstrate the value of addressing barriers to health. The team will highlight exciting components of Rosewood’s alternative model as well as discuss tools like Arcadia that will help them to push innovative and informed care even further. Arcadia account manager Eliza White will join the YVFWC speakers to share more about how YVFWC is utilizing Arcadia Analytics across their organization
